New Author Form for 2013 to prepare for the IFLA repository

In 2013 authors of WLIC papers are being asked to sign a new version of the Author Form which will assign a license to their paper allowing IFLA and others to make copies freely available. The requested license aligns with IFLA's Open Access policy and gives IFLA permission to upload the papers to the new repository.
